Example forms for Head Coaches

If you attended Ruth's New Head Coach Workshop, you saw examples of several forms that she uses to help organize and run her elementary team. (Click here to download the slide presentation)

You can click links below to open a .pdf file to view and print,
or editable files in Microsoft Word 97-2003 (.doc) and Excel 97-2003 (.xls)

There's many ways to run your program- you decide what fits your style.

These example forms are provided for your reference only.

Use what you like- change what you don't like- or ignore them all and create your own- you decide! You can contact Ruth at This email address is being protected from spambots. You need JavaScript enabled to view it.

Congratulations, you've made the team

I conduct an after school Science Club in September. Students that participated in the club are eligible to try out for the Science Olympiad team. They take a general science knowledge test, and their teacher is consulted about their attitude and study habits.
The students chosen for the team receive an envelope containing this congratulations letter and a "Science is a Blast" button. Click for a closer look!
The envelope is presented by their classroom teacher at the end of the day Friday, to help minimize the impact on the students that didn't make the team.

Sorry, thanks for trying

There may not be room for everyone that tried out for the team.

I teach in a K-5 school.There is a different letter for 4th and 5th graders because...

I ask their classroom teacher to hold on to the envelopes until the end of the day Friday. Some students need the weekend to get their emotions under control.Click for a closer look!

In the envelope, I include the same "Science is a Blast" button that the successful team members receive.

Commitment to the team

It's important for students to understand that being on the Science Olympiad team is a long term commitment, requiring continued work and study well into the Spring when the weather may make it difficult to stay focused.

Student Event Checklist

Students spend time in advance of the final event selection meeting thinking about their first, second and third choice.

Everyone isn't guaranteed to get their first pick- It's been my experience that students who take on events they didn't think they were interested in quickly become enthusiastic when they get started and learn more about it (because that's the type of kid that tries out for the team)

Parent Info Letter

It's better to receive some information directly from the parents- not only to insure accuracy, but to be sure the parents are involved and aware.

When an event coach is chosen...

The coach provides some basic information about when and where they plan to hold practices. This isn't only to collect information from the event coaches- it insures these important details are under control and is a good way to initiate discussion/clarification with the Head Coach.

A little extra help

There are a few activities that don't require the long time commitment of an event coach, but probably don't need to be added to the Head Coaches busy schedule.
